Non-School Transportation from District Sponsored Events Authorization/ReleaseThis form is used to permit parents to authorize their student to be transported from certain school and/or extra-curricular events by persons the parents have approved in writing with respect to that event. This form MUST BE SIGNED IN THE PRESENCE of a school official as noted on the form. |
